For New Patients
All new patients are required to answer the questions on our new patient forms. Please come 15 minutes prior to your appointment to fill out the forms. Or download the forms from below link, print, fill out beforehand and bring it with you. (ODF file)
On Your Initial Visit
- Please bring a valid ID (driver’s license or passport) and insurance card.
- For payments, cash, check, and card are accepted.
- Please do not wear clothing that has metal, such as zippers, since it will interfere with your X-ray. Please feel free to bring a change of clothes to the clinic, if needed.
- If you must cancel or reschedule, please contact us as soon as possible. If you no show without contacting us, we may charge a cancellation fee.
